Success Story

Taking to the Streets

Monthly LCHC health coordinators provide services to the homeless community throughout the city.

LCHC In The Community

Meeting the Needs of the Underserved

Monthly LCHC’s health coordinators equipped with mobile tool kits and a heart for service canvass neighborhood streets and parks eager to meet the needs of the District’s underserved population.  Whether at 14th Street and Franklin Park, the Second and D Street Homeless Shelter or random corners throughout the city; LCHC and its partner organizations have served over 5,611 meals to the homeless; provided clothing, toiletries, and referrals to housing, employment, mental health care, substance abuse treatment and other supportive services.

Our health coordinators are committed to meeting the needs of the city’s homeless whereever, whenever and however possible.  Engaging the underserved where they are in order to build relationships, gain trust and create an environment to connect those who are not connected to the health and social services that eliminate barriers to healthy lifestyles.

We are looking for partners to continue on this journey with us to make lasting impacts for underserved individuals.
